As part of our initial consulta­tion, we like to take the oppor­tunity to get to know each mem­ber of the couple in a calm and relaxed atmo­sphere so that we can get a full view of their per­sonal situa­tion and health status.

Please allow about 1 hour of time for the initial con­sulta­tion.

The best way to schedule an initial consulta­tion is by phone (Monday to Friday, from 8:00 a.m. to 3:00 p.m.).

Prior test results

If you already have documentation concer­ning prior examina­tions and/or prior treat­ments, please send us a copy in ad­vance by post:

  • Basal hormone status of the woman (blood sample on 2nd to 5th day of cycle)
  • Spermiogram of the man
  • All documents on prior treat­ments or opera­tions
  • Information on other health prob­lems
  • Possibly older laboratory find­ings, if relevant

Providing information & results

Please send us the completed PDF form and copies of any exis­ting test re­sults by post at least one week before your appoint­ment. Please note that, for data security reasons, the use of email is not suitable for trans­­mitting health-related data.

Please complete the PDF form in full and legi­bly. All fields marked with * are manda­tory fields. We need this informa­tion so that we have a record of your data at our Center before your appoint­­ment; this en­ables us to begin your ini­tial con­sulta­tion on time. Thank you.
